Through CEBT, we offer a comprehensive vision insurance plan (Vision C) available to all eligible employees. VSP is our carrier for Vision C.

No ID Card? No Problem. You do not need a physical ID card to receive services. Simply tell your provider you are covered under VSP, and they can look you up in their system using your name and date of birth.

Coverage & Benefit Frequency

  • Your vision benefits reset every 12 months for exams, lenses, and frames.
  • Routine WellVision Exam: $10 Copay
  • Lenses (per pair): $10 Copay (Includes single vision, bifocal, trifocal, and lenticular lenses)
  • Frames Allowance: $175 allowance for a wide selection of frames ($195 allowance for featured frame brands).
  • Contact Lens Allowance: $175 (in lieu of glasses).
  • Routine Retinal Screening: No more than a $39 copay when added to a routine eye exam.
  • Lens Enhancements: Standard progressive lenses are covered in full; Premium and Custom progressive lenses have a $95–$175 copay.

Extra Savings on Frames: Get an extra $20 to spend on featured frame brands like Bebe, Calvin Klein, Nike, and more.

Glasses and Sunglasses: Receive a 20% discount on additional pairs of prescription glasses and non-prescription sunglasses from any VSP provider within 12 months of your last exam.

Laser Vision Correction: Average 15% off the regular price or 5% off the promotional price at contracted facilities.


Exclusions: Benefits covered under Workers Compensation Act, surgery or medical treatment of eyes, replacement of lost, stolen, or broken lenses and / or frames, services, and supplies for which you or your dependent are not required to pay, services and supplies not listed.

An employer must have at least 25% of the eligible employees enrolled in the plan in order to offer coverage.
