Data Privacy Consent
Weld RE-4 School District is committed to providing students with the best in educational resources and technology to aid students’ learning. During the school year, your child’s school will utilize a variety of websites and web-based applications to supplement curriculum and provide interventions. Depending on which school your child attends, your child will have access to and will be encouraged to use these resources.
The School’s use of websites and web-based applications is subject to two important privacy laws, the Family Educational Rights and Privacy Act (“FERPA”) and the Children’s Online Privacy Protection Act (“COPPA”). FERPA prohibits the disclosure of personally identifiable information (“PII”) from students’ education records, including student class work or assignments that are stored on certain websites, without consent of parents/legal guardians or eligible students, except in limited circumstances. COPPA requires operators of websites or online services to get parental consent before collecting any personal information from children under 13 years of age.
Students using these websites and online services often must create an account and provide information upon login, including their first name, last name, grade level and username. In addition, students may store school work within these websites and applications or be asked to directly or indirectly share PII. In some cases, these websites or online services may passively collect PII from students, including those under the 13 years of age. As such, the school requires written parent/guardian consent for students to use these sites and applications. Parent/guardian consent is voluntary. However, students who do not have parent/guardian consent to use the sites and applications will be unable to access or participate in some of the activities conducted online.
The school may use any website or application listed on the District’s Student Data Privacy page (https://weldre4.org/departments/technology/student_data_privacy) under the tab “On-Demand Providers”. This list may be revised at the beginning of or during each term. The District will not provide specific notice to parents/guardians when this site is updated. You are encouraged to visit this site as frequently as you deem necessary.
By clicking “Agree” below and signing this registration form, you certify that you have reviewed the list of sites and applications indicated for your child’s school, that you have been informed that each company maintains a Privacy Policy and, to the extent you have deemed it necessary, you have reviewed such policies before signing below. You acknowledge that this list of websites and applications may change from time to time and that this consent form applies to the websites and applications listed at the time you sign this form and any hereafter added to this list. You acknowledge that you have been informed of where to obtain information about these sites and applications. You understand that the sites and applications listed may collect PII pertaining to your child for the purposes of creating and logging into an account maintained by such site or application or as otherwise described in each company’s Privacy Policy. You give your consent for your child to use the listed sites and applications for school-related purposes for the upcoming school year.
You acknowledge that this consent can be revoked at any time by providing written notice to the front office of your child’s school and that such withdrawal of consent will result in the termination of your child’s access to these websites and applications. Notwithstanding the foregoing, you acknowledge that terminating your student’s ongoing use of such websites or applications will not be effective in removing information your child previously provided such website or application and that if you so desire, such request must be undertaken in accordance with each company’s Privacy Policy, of which you have been informed.
