Managing a child's seizure disorder or epilepsy is a team effort, involving you, your child, your child's healthcare providers, and the school. All members of the team want to be able to communicate, ask questions, and share information about your child's health and well-being.

Please complete the Questionnaire for Parents of a Student with Seizures (English | Spanish) for your school nurse. This questionnaire only needs to be completed once, or as your student’s seizure condition changes.

All students with a diagnosis of a seizure disorder / epilepsy, must have a Seizure Emergency Care Plan. Most neurologists will provide you with a pre-printed Seizure Emergency Care Plan. This care plan must be updated annually. If your doctor does not have a pre-printed plan, use the Seizure Emergency Care Plan and Medication Orders Template.
